Full Fellowship – Pharmacist Member (Hospital, Academia, Association) Application

The membership fee is for one year (12 months) of membership. If a membership is cancelled before the expiration date, the individual or corporation can still access membership advantages until the end of the 12 month term. However, the individual or corporation is still responsible for the entire annual fee, whether paid in a single annual payment or through the 12  month installment plan.  If a cancellation is made and a refund is requested after a membership renewal processes, a 5% administrative fee will be charged.
